docs: rewrite AGENTS.md — dedupe fleet history into lessons + runbook index

- 734 -> 412 lines; active topology table replaces the swap-history log
- retired-runtime detail collapsed into a runbook pointer table
  (serverAppConfigs-docs/LLM/ holds the full runbooks)
- halogen section condensed to invariants that survive config changes
  (pool/reserve matched pair, free-lies rule, client contract)
- new Key Lessons Learned section: ROCm/layer-split, HIP env collisions,
  mmap/PLE constraints, crash diagnosis, swap ordering, podman/quadlet
  gotchas, ops guards
- fixed the merged/broken row in the Summary table
