Adds Rule 6 — Keep the Todo List LIVE, Not Retrospective to agent/APPEND_SYSTEM.md (the global behavior file injected into every omp session, every cwd — verified live: it is the harness-side persona/rules append, ~/.claude/CLAUDE.md user-scope does not exist on this box).
Motivation (Frank, 2026-10-18): tracking often flips only when the entire task bill finishes — the list is a history dump while work runs. Rule pins the mechanics the harness supports: init before first substantive step; done+start batched with the step they describe; block + reason for stalls; no lone todo calls.
Tests: bats tests/ 36/36 (docs_completeness unaffected — no new pages).
